USA PATRIOT ACT

In accordance with federal regulations, Connected Processing Services, LLC, will comply with Section 326 of the USA PATRIOT ACT, known as the Customer Identification Program (CIP) to help prevent money laundering, terrorist financing and various forms of fraud, such as identity theft.

The purpose of the ACT and its procedures are to insure, to the extent possible, that requirements are made of the financial institutions to obtain more information from an individual or legal entity to help establish identity.

Your cooperation is needed when you open a new account with Connected Processing Services, LLC.

IMPORTANT INFORMATION ABOUT PROCEDURES FOR CREATING A NEW ACCOUNT

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.
What this means to you:
When you open an account, we will ask for your

• name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
  You may be asked more questions to establish and confirm your identity, and may be required to provide one or more types of identification.
• Two (2) forms of Proper ID are required: Driver's License (unexpired)
  Passport and country of issuance
  U.S. Taxpayer Identification (ID number) Alien ID Card
• Any other government-issued document evidencing nationality or residence and bearing a photograph or similar safeguard. Connected Processing Services, LLC, will keep the required document records for CIP purposes, while ensuring that such information is not used in violation of Federal or State law.
